This form should be completed only by those publishers requiring EAN-13 bar codes for existing ISBN Publisher Prefixes. If you are a new publisher needing ISBNs, please visit our website at www.isbn.org to obtain an ISBN Publisher Prefix Application form and information. Publishers applying on isbn.org for the first time will have the opportunity to request EAN-13 bar codes for their new ISBN Publisher Prefix assignment.

Please supply the complete 10-digit ISBN(s) you are requesting to be converted into the EAN-13  bar code. Please ensure the ISBN(s) is taken directly from your ISBN list supplied by the U.S. ISBN Agency. The Agency is not responsible for inaccurate or invalid ISBN supplied by a publisher. 

(If you have not set a price for your publication at this stage, the price add-on on your bar code will read 90000. This 90000 price extension is a “dummy code” that allows bookstores to manually enter the price of the book).
